Here is a full statement released by a family member:
"It is with shattered hearts we type this message. Jake left us to go home to be with his Lord and Savior. He will remain in the hospital for a few more days because we are honoring his wishes. When we took Jake to get his DL he came to the organ donor question and asked us what to check. We told him that was his decision to make. Without hesitation our sweet angel boy checked that box. Leave it to our sweet, thoughtful, loving baby boy to find a way to continue giving even after he has left us. We take great comfort in knowing that he is with his Papa Jake and they are dancing with Jesus.
Thank you all for your prayers, messages, posts, hugs, and support. We can feel the love of our beautiful community.
Once arrangements are made we will share them on Facebook.
We love you all."
